When you visit your dentist take your claims form with you, along with your policy wording and benefit schedule.

How to make a claim on your Boots Dental Insurance Plan
- Ask your dentist to complete their section of the claims form.
- Request a dated receipt once you've received and paid for your treatment, so that we can reinburse you.
- Complete, sign and date your section of the claims form.
- Attach the payment receipts to the form and return it to us within 90 days of your treatment date.

Alternatively, send your completed claim form by email with scanned copies of necessary documents to Boots@denisglobal.com. Your claim will be assessed and you and your dentist may be asked for any missing or subsequent information.
If your claim is eligible for settlement, a cheque will be despatched or the settlement paid directly into your account according to your instructions stated on your claim form.
