To enable us to provide you with this service and to give you the best and most appropriate care and advice, we will collect your personal data such as name, address and date of birth; we also collect your more sensitive personal data regarding health such as your medical and family history for the purposes of ensuring the service we provide is appropriate for you.
Your personal data will be stored for as long as necessary unless a longer retention period is required or permitted by law.

How will the NHS Electronic Prescription Service help me?You may find this service particularly beneficial if you receive regular prescriptions. This service will mean you don't have to collect your repeat prescription from your GP's surgery between check-ups.
Your GP or pharmacist will be able to tell you where the service is already being offered.
The service can be used for:
- Repeat prescriptions in England
- One-off prescriptions in England
The service cannot be used for:
- Private prescriptions
- Veterinary prescriptions
You can sign up to our free repeat prescription service. This means your medicines will be ready and waiting for you to collect at your pharmacy. Sign up online here or in-store.
The service is being rolled out at GPs surgeries and pharmacies in England and many people have already signed up to it.
Your GP or pharmacist will be able to tell you where the service is already being offered.
All Boots pharmacies in England are already set up to receive electronic prescriptions.
The service can be used for:
- NHS prescriptions in England
The service cannot be used for:
- Private prescriptions
- Veterinary prescriptions
If your GP isn't offering the service yet, you can still sign up. If you choose Boots we will keep your completed NHS Electronic Prescription Service form, so you're ready to start using the service as soon as it's available at your surgery.
If you have moved house, or want to change the Boots store your electronic prescriptions go to, simply enter the new details using the form above, click submit and your nomination will be changed. Alternatively, ask in store for your nomination to be changed.
Yes, you can sign up for your partner, children or other family members, as long as you live with them and you all want to use the same Boots pharmacy. Just select the option to choose a pharmacy on behalf of someone else on the form.
