In using Boots Health Assessments you are deemed to have read, understood and accepted all the Terms & Conditions.

This service is offered by Boots. As our appointed partner, roadtohealth will hold your information and then pass this information back to us.

Boots Health Assessments are subject to availability; we may change or remove all or any part of the Boots Health Assessments product at any time. Any changes will be posted in the terms and condition section of www.boots.com.

Information you submit as part of a Boots Health Assessment will be stored securely on our behalf by roadtohealth, who will pass this information back to us from time-to-time through an encrypted secure connection. Your information may be used to provide you with additional health services from Alliance Boots but we will not sell this information, nor will we share this with an organisation other than those who require it to provide services for you on behalf of Alliance Boots.

Boots Health Assessments are provided to give you medical, health, fitness or similar information based upon details you provide to us about your lifestyle preferences, family and clinical history and your health aspirations and is intended for you to use solely as an educational aid. The information is in no way designed to substitute the expertise and judgment of your doctor, pharmacist or other healthcare professional. You are advised that decisions regarding the diagnosis and treatment of any medical condition, including decisions regarding drug therapy, are complex medical decisions requiring the independent and informed judgment of your appropriate healthcare professional. All specific questions regarding the treatment and care of a medical condition should be posed to your own professional healthcare provider.

If you have any concerns after carrying out a health assessment you should talk to your doctor.

By using Boots Health Assessments, you confirm you have understood and agree to be bound by the following terms and conditions:

roadtohealth controls and operates this website from the United Kingdom and the information delivered via the website is deemed to have been delivered in the United Kingdom. This website is not directed to any person where (by reason of that person's nationality, residence or otherwise) the publication or availability of this website is prohibited. Persons to whom such restrictions apply must not access this website. If you choose to access this site from outside the United Kingdom you are solely responsible for compliance with local laws.

What type of data we will collect from you

When you use Boots Health Assessments, roadtohealth may process sensitive or personal data about you. This may include:

• Details about any of your previous health concerns or clinical issues;
• Details about your family history, particularly around health concerns or clinical issues;
• Details about your lifestyle; and
• Clinical information.

We will only collect data to the degree that this is necessary and relevant to the delivery of Boots Health Assessments or for any future products or services Boots may offer you from time-to-time.

How we will use your data

The information you provide us with will be used predominantly to provide you with information about a number of potential health risks based on the information that you provide to us. In addition your data may be used to:

• tailor your visit to the Website;
• show you content that we think you might be interested in;
• contact you and send you information regarding our services and changes to our services;

• contact you if we need to resolve support issues;

How the Website uses Cookies

How to opt out of future mailings and communications

To change your communications preferences for from Boots Health Assessments simply logon to Boots.com, select My Account, My Healthcare Account and select Health Record. In Health Record you will find Communications Preferences section, simply select edit and change your preferences.

How we will keep your data safe

Boots and roadtohealth take your security and privacy very seriously and use access authentication and server security to protect your data. Whenever you provide information to Boots Health Assessments across the web, your details will be encrypted before being sent to professionally managed Website servers.

Please note that Boots and roadtohealth cannot be held responsible for security issues generated by equipment you use to access Boots Health Assessments, or certain circumstances beyond their control including but not limited to hack attacks, denial of service attacks, the effect of viruses or other harmful or malicious programs or vandalism of services.

Your data will be held by roadtohealth in a purpose-built, professionally managed, secure data storage facility in the UK and will be monitored 24 hours a day, 365 days of the year. If, for any reason, roadtohealth need to transfer your information to any other country, which operates to a different level of data protection from the UK, they will always put a contract in place to ensure your information is protected.

How we comply with the Data Protection Act 1998

roadtohealth have internal procedures to ensure that all information which is collected and held about you is held in accordance with the legal requirements and principles of the Data Protection Act 1998. The Data Protection Act 1998 has eight main principles these hare listed below together with an explanation of how roadtohealth comply with these principles.

Both Boots and roadtohealth recommend that you review the paragraphs on Privacy and Security above which set out the measures, which have taken to protect your data.

Personal data shall not be transferred to a country or territory outside the European Economic Area (EEA) unless that country or territory ensures an adequate level of protection for the rights and freedoms of data subjects in relation to the processing of personal data.

The UK has a high degree of data protection regulation and consequently you can be assured that wherever you are, the fact that all of your data is stored on servers based in the UK means that your information is processed in accordance with UK data protection principles. If you access your e-mails from a computer located outside the EEA, the action of accessing your emails will be taken as your consent to us transferring your personal data outside of the EEA in order to reach you.
