This service is offered by Boots. As our appointed partner, roadtohealth will hold your information and then pass this information back to us.
Boots Health Assessments are subject to availability; we may change or remove all or any part of the Boots Health Assessments product at any time. Any changes will be posted in the terms and condition section of www.boots.com.
Information you submit as part of a Boots Health Assessment will be stored securely on our behalf by roadtohealth, who will pass this information back to us from time-to-time through an encrypted secure connection. Your information may be used to provide you with additional health services from Alliance Boots but we will not sell this information, nor will we share this with an organisation other than those who require it to provide services for you on behalf of Alliance Boots.
Boots Health Assessments are provided to give you medical, health, fitness or similar information based upon details you provide to us about your lifestyle preferences, family and clinical history and your health aspirations and is intended for you to use solely as an educational aid. The information is in no way designed to substitute the expertise and judgment of your doctor, pharmacist or other healthcare professional. You are advised that decisions regarding the diagnosis and treatment of any medical condition, including decisions regarding drug therapy, are complex medical decisions requiring the independent and informed judgment of your appropriate healthcare professional.
