How to make a claim on your Boots Dental Insurance Plan
- Ask your dentist to complete their section of the claims form
- Request a dated receipt once you’ve received and paid for your treatment, so that we can reimburse you
- Complete, sign and date your section of the claims form
- Attach the payment receipts to the form and return it to us within 90 days of your treatment date

Alternatively send your completed claim form by email with scanned copies of necessary documents to Boots@denisglobal.com.
Your claim will be assessed and you or your dentist may be asked for any missing or subsequent information.
If your claim is eligible for settlement, a cheque will be dispatched or the settlement paid directly into your account (Direct Debit customers only).
