The NHS Organ Donor Register, is a vital resource that helps save and improve lives, you can join by applying for an Advantage Card or as an existing cardholder.
It is a confidential, computerised database, which holds the wishes of more than 18 million people. They have all decided they want to leave a legacy of life for others. Adding your name to the register is the most reliable way of expressing your wishes but it's really important you also tell your relatives what your wishes are.
You can find out more information about the NHS Organ Donor Register by visiting www.organdonation.nhs.uk or calling NHS Blood and Transplant on 0300 123 23 23.
Would you take an organ if you needed one? Nearly everyone would. But only 29% of us have joined the NHS Organ Donor Register. More than 10,000 people in the UK currently need a transplant. Of these, 1,000 each year - that's three a day - will die waiting because there are not enough organs available. Please help us turn people's good intentions about organ donation into action by registering on the NHS Organ Donor Register. Between April 1 2009 and March 31 2010, 3,709 organ transplants were carried out across the UK thanks to the generosity of 2,021 donors.
If you don't have an Advantage Card already, you can choose to register as an organ donor when you apply for your Advantage Card. If you're applying online or in store, make sure you tick the relevant box on the application form. You can also apply over the phone by calling 0845 124 4545* - ask to sign-up to the NHS Organ Donor Register at the same time as applying for your Advantage Card. When you receive your Advantage Card, the Organ Donor symbol will be printed on the back of the card. If you already have an Advantage Card, please contact the Advantage Card team on 0845 124 4545* and let them know you would like to be added to the register.
If you want to remove yourself from the NHS Organ Donor Register, please contact 0300 123 23 23 or go to our website http://www.organdonation.nhs.uk/register and fill in the form asking for your name to be removed. If you would also like the symbol removing from your Advantage Card please contact 0845 124 4545*.

Justine Laymond, 38, from Essex, who received a double lung transplant from the NHS Organ Donor Register, explains why signing up is such an important thing to do.
"I was 31 when I was diagnosed with a rare lung disease and given just 12 to 18 months to live," she says. "I went from being a young woman with everything to live for, to being just hours away from death. "By the time the call finally came, over a year later, to say that the register had found two lungs for me, Mum said I was dying in front of her eyes. Now I can breathe properly and run again - in October 2009 I successfully completed the 10-mile Great South Run - and all thanks to the NHS Organ Donor Register.
"I want to say a massive thank you to the 1.25 million Advantage Card holders who've joined the register using their Advantage Card. By adding your name to it, you're helping to save lives just like mine."
